We present the therapeutic management of a patient initially diagnosed with inverted papilloma, originating in the left sphenoid sinus, that subsequently maligned.

We emphasize a few particularities of the endoscopic endonasal approach: timing of the surgery, anatomical and surgical risk elements, complete resection difficulties.

Este prezentată conduita terapeutică urmată în cazul unui pacient diagnosticat iniţial cu papilom inversat cu origine la nivelul sinusului sfenoid stâng, ulterior malignizat, punând accentul pe câteva particularităţi ale abordului chirurgical endoscopic endonazal — moment operator, elemente de risc anatomochirurgical, dificultatea obţinerii unei rezecţii complete.
